Reports confirm that the accident happened on Friday, on Interstate 79 in New Vernon Township at 8:04 PM. Pennsylvania State Police said that the college student was a passenger in a minivan. 18-year-old Matteo A. Guajardo was reportedly driving the vehicle. It was heading south on I-79 when the driver lost control. While on road, the vehicle traveled off the east side of the road. Later, it went up and over an embankment and eventually, rolled over.

With its wheels facing east, the minivan came to rest in the left lane of northbound I-79. The accident claimed the life of the young student. The minivan was blocking the left lane, according to the police, and its lights had failed. At that moment, a truck being driven by Cochranton resident Daniel D. Byler, 51, struck the passenger side of the minivan. The truck and minivan both stopped off the road. Guajardo was transported in an ambulance to a local hospital with what are believed to be significant wounds.

Byler was unharmed. According to the police, Guajardo was not wearing a seatbelt, but Duncan and Byler were.
